2026-27 Athletic Paperwork

We have gone to an online platform to submitt paperwork.  We will no longer accept hard copies of paperwork.  Please see the attached flyer for directions on filling out the paperwork. If you have any questions, please email Michele Copello.

Make sure you have the following documents saved as a PDF file to upload into the clearinghouse portal.

1.  EL2 pg. 4 (physical form) – this is the only REQUIRED physical form to submit.  Please make sure that all sections are completed FULLY before uploading.  Physician credentials must be provided on the form in either written or stamped form.

2.  Watch and download the video certificates for Concussions in Sports, Sudden Cardiac Arrest, Heat Illness Prevention and Sportsmanship.  Links to the video website (nfhslearn.com) can be found to the right.

3.  Mandatory insurance link can be found to the right.  The insurance offered on the athletic clearance site IS NOT the mandatory insurance.  You can begin to purchase insurance for 2026-27 on 7/1/26, HOWEVER the insurance will NOT begin coverage until 7/27/26 or date of purchase after that date.  If you need insurance to cover you for July, you will need to call the insurance company and add the summer participant insurance.

4.  NEW REQUIREMENT:  Beginning 7/1/26, the EL1 form and any supporting documents required for the EKG is required for 1st time FHSAA athletes.  This is a one-time requirement for high school students.  EKG’s dated 7/1/24 or later are acceptable for the 2026-27 school year.  The EL1 form can be found to the right.

5.  GPA requirement:  In order to participate in FHSAA sports, a student must have an unweighted GPA of 2.0 by the beginning of the first day of school.  Ninth graders have a waiver for the first semester and will begin their GPA requirement in the spring semester of their ninth grade year.

6.  Birth certificate is required for incoming students (incoming 9th graders, co-enrolled or transfer students).

7.  There may be other paperwork required for incoming students that are co-enrolled or transfer.  There are spots for those file uploads as well.

Please contact the coach of the sport for information pertaining to the particular sport you are interested in.  A complete list of coaches is provided.  Many coaches have an email blast that they send out for communication.  Please contact the coach to get on their email list.

NOTE:  Band paperwork will NOT be accepted in lieu of Athletics paperwork. The forms are NOT the same. If a student is participating in both Band and a Sport, he/she must fill out separate packets. Students must have ALL of the above completed to even do summer conditioning.

If you have any questions, please email the Athletic Coordinator, Michele Copello:copellom@pcsb.org
